Dr. Butuk has been practicing family medicine since 1994.
He graduated from medical school in 1991 and completed his family practice residency in 1994. Dr. Butuk is board certified by the American Board of Family Practice and is a member of the American Board of Family Physicians and the Idaho Medical Association.
Dr. Butuk established Meridian Family Medicine in 2006 to create a doctor owned medical practice with the ability to provide every patient with first-rate medical care. Dr. Butuk focuses on preventive medicine with an emphasis on healthy nutrition and lifestyle.
Dr. Butuk, his wife and two children live in Meridian. He enjoys a busy and active lifestyle outside the clinic.
Leanna Moser, MSN, FNP-C, PHN
Certified Family Nurse Practitioner
Leanna joins the practice as a graduate with honors from Walden University with a fresh and current perspective regarding best health practices. She has been in health care since 2005 working as a Registered Nurse in Women and Infant Care and as an emergency room nurse in the second busiest Emergency/Trauma Center in Los Angeles County, California. Leanna has a passion for providing excellent care and self-care teaching support to patients of all ages, but especially enjoys the specialized care she can offer regarding women’s health issues, Diabetes/Metabolic diseases and mental health disorders. She is a champion for groups often frustrated by progressive and serious disease processes, as well as the disenfranchised populations among us.
Leanna and her husband, a law enforcement officer, recently moved to Idaho with their four children in pursuit of new adventures and an opportunity to enjoy the great outdoors! She enjoys camping, traveling, hiking, rafting and horseback riding, although her greatest satisfaction comes from helping patients and their families to achieve quality care.
Meredith Mangum, FNP-C
Certified Family Nurse Practitioner
Meredith brings to Meridian Family Medicine over eight years of healthcare experience. She joins the practice as a recent graduate from Frontier Nursing University, where she represented her class as student body president. In 2008, she became a Registered Nurse after graduating from Boise State University, where she played soccer for the Broncos. She has worked in emergency rooms and intensive care units for various hospitals across the Treasure Valley, as well as a weight loss surgery clinic. She became a Family Nurse Practitioner to provide holistic and preventive care for families. Her personal philosophy of mindfulness fuels her enthusiasm to treat every patient with empathy and compassion. She welcomes all patients, but is particularly interested in weight loss therapies, women’s health, and treating depression and anxiety. As a former collegiate athlete, she also has a passion for sports medicine and promoting health through nutrition and exercise.
Born and raised in Boise, Meredith is a true Idaho native. An outdoor enthusiast, she enjoys snow skiing, whitewater rafting, running, and yoga. She loves attending musical concerts, but also spends quiet time making jewelry. Creating meaningful relationships with her family and friends has always been a priority, and she is now looking forward to building lasting relationships with her patients at Meridian Family Medicine.
Scott Frisby, PA-C
Certified Physician Assistant
Scott comes to Meridian Family Medicine with over twelve years of experience in healthcare, where he has focused primarily in urgent care and family practice. A graduate with a Masters of Physician Assistant Studies from Rocky Mountain College, he worked as the general surgery team leader in Bozeman, Montana before moving to Utah and overseeing the diagnosis and management of acute and chronic conditions for all age groups at the practice where he worked. Eventually settling in Meridian, Scott worked at an urgent care where he handled minor emergencies, fracture management, laceration repair among other physician assistant duties.
Scott's personal philosophy of quality instruction with compassion allows him to provide personalized care for patients of all ages and backgrounds. As an avid outdoorsman, Scott enjoys hiking and camping where he loves to capture his beautiful surroundings with his camera. He is a black belt in Tae Kwon Do and plays jazz string bass in his spare time.
His wife and four boys are his life and he is excited to work at Meridian Family Medicine where he can build relationships with patients and do what he loves.
